Can someone share a sample or a template for the affidavits from former colleagues / managers, in cases where employer experience letter in specified format is not obtainable?

Here it is. I used this format for my I-140. Got approved without RFE. It was on plain white paper without any letter head or such.
Date:
To Whomsoever It May Concern
This letter is to certify that Mr.XXXXXX worked as a < Job title> from < date> to <Data> at <Company Name> Located at < Address >
I worked as Programmer Analyst at <Name of the Company> , <Address > from (Start date of Employment- Month/Year) to (Last date of employment, Month/Year). As a co-worker of Mr. XXXXX, I have knowledge regarding XXXXX's job duties, dates of employment and hours of work.
YYYY Company is in the business of providing software consulting services.
XXXXXX's duties included design, develop and administer Oracle databases, perform backup and recovery procedures. Design, develop and test custom applications using Oracle Pl/SQL
Mr. XXXXXX worked for 40 Hours per week.
Please feel free to contact us if you require any further information.

Here is my question. In my college days bacd in the early 90's and mid 90's, I used to work and worked with my original Social secuirty number. Another words, I was given pay checks. I paid taxes and got refunds as well. Obviouslly all illegal since I had no permission to work as I was on FI visa. Anyways, after graduating college, I got H1b visa. Now I am in my 9th year, and my labor and I140 have already been approved. So the question is, when I am called for my 485 interview, will they know that i worked illegally, will they have records of that. I also have a DWI. I think DWI is ok, as I have read many got their green cards after this. I am more concerned with my working illegally.
Just recentlly I was sued by Discover card. The judgement against me in teh court was for 10,000 +. I have paid half of it and will pay the rest in 6 months. Will that affect my Green Card.
Please eloborate.

Are you serious? I must be a super expert on taxes because I have always done it on paper using a calculator for the last 14 years and counting. Just spend an hour reading the instructions and you're done. Have a question, call the IRS. Unlike other government agencies (cough.. USCIS cough..) the agents at the other end of the line are actually helpful and responsive.

Well, Not every one is as patient as you are in reading the instructions and if needed a friendly manual or dialing 800 829 1040. The feeling is mutual about the IRS helpline folks.
The OP seemed very confused and thats why I suggested not to fill in on paper by hand.
